Aging Resources

  • Alpine Area Agency on Aging – www.AlpineAAA.org
    Alpine Area Agency on Aging provides and connects our community with regional resources, supports, and services that promote aging with independence and dignity for individuals sixty and older and their caregivers in their community of choice.
  • Colorado State Unit on Aging – State of Colorado Unit on Aging
    The State Unit on Aging programs consists of the following programs: Nutrition, Supportive Services, Caregiver, ADRC, and Senior Community Service Employment Program.
  • National Administration on Aging – www.aoa.gov
    The Administration on Aging (AOA) is the principal agency of the U.S Department of Health and Human Services designated to carry out the provisions of the Older Americans Act of 1965 (OAA), as amended (42 U.S.C.A. § 3001 et seq.). The OAA promotes the well-being of older individuals by providing services and programs designed to help them live independently in their homes and communities. The Act also empowers the federal government to distribute funds to the states for supportive services for individuals over the age of 60.

Disability Resources

  • Horizons Specialized Services – www.horizonsnwc.org
    Horizons Specialized Services was created in 1975 to provide services to individuals with developmental disabilities in five northwest counties of Colorado: Grand, Jackson, Moffat, Rio Blanco and Routt. Services in Grand County are primarily home based.
  • Northwest Colorado Center for Independence – www.nwcci.org
    Services and supports for people with disabilities in Routt, Moffat, Summit, Rio Blanco, and Grand Counties. Core services include information and assistance, advocacy, peer support, youth transition services and independent living skills. We work with individuals of all ages and disabilities so that everyone can choose to live well in the community.
  • National Sports Center for the Disabled – www.nscd.org
    The NSCD offers a variety of year round sports and recreational adventures for persons with disabilities. All programs are designed for individuals, families or groups and are available for all disabilities, diagnosis, and ages.

    Fixed route transportation services available in Fraser and Winter Park. Buses are wheelchair accessible and para-tranist is available.
